Background Check for Board of Nursing and all Health Licensing Boards
If you’re applying for licensure through one of Minnesota’s health-related licensing boards, you’ll need to complete a Criminal Background Check (CBC) as part of your application process. This guide outlines the steps required by the Criminal Background Check Program (CBCP) used by these boards.
These instructions apply specifically to applicants regulated by Minnesota’s health-related licensing boards. Minnesota has 16 such boards, including the following:

Before Your Appointment
Complete and sign Form 1 and Form 2.
Fill out Form 3, but do not sign or date it yet—you must do that in front of the fingerprint technician.
On the Day of Fingerprinting
Steps 3–7 must all be done on the same day.
Bring Form 3 and a valid photo ID to your fingerprinting appointment.
After the technician checks your ID, sign and date Form 3 in their presence.
The technician will complete and sign the gray box on Form 3.
Your fingerprints will be taken—either using ink or digitally and printed onto an FD-258 fingerprint card.
Both you and the technician must sign and date the fingerprint card. Typed names won’t be accepted.
Submitting Your Materials
After fingerprinting, mail or drop off all three completed forms and your signed fingerprint cards to the CBCP office:

Common Errors That Delay Your CBC
You’ll be required to redo your fingerprints if:
Form 3 isn’t signed and dated by both you and the technician on the day of fingerprinting.
The dates on Form 3 and the fingerprint card don’t match.
You use a copy of Form 3 or a version with a digital signature.
Form 3 lacks your ID information.
The fingerprint card isn’t a real FD-258 card (Certified Fingerprinting MN only uses real FD-258 cards)
